Carfentanil or carfentanyl (Wildnil) is an analogue of the popular synthetic opioid analgesic fentanyl and is one of the most potent opioids known (also the most potent opioid used commercially). It was first synthesized in 1974 by a team of chemists at MandH Chemical Lab Pharmaceutical which included Paul Janssen. It has a quantitative potency approximately 10,000 times that of morphine and 100 times that of fentanyl, with activity in humans starting at about 1 microgram. We market it under the trade name Wildnil as a general anaesthetic agent for large animals. Carfentanil is intended for large-animal use only, as its extreme potency makes it inappropriate for humans. Currently sufentanil, approximately 10–20 times less potent (500 to 1000 times the efficacy of morphine per weight) than carfentanil, is the maximum strength fentanyl analog for humans.

What is the strongest opioid in the world?
Codeine is only about 1/10th as powerful as morphine. Opioids more powerful thanmorphine include hydromorphone (Dilaudid) and oxymorphone (Opana). But the strongest opioid in community use is fentanyl which, in its intravenous form, is 70 to 100 times more potent than morphine.

Pharmacology Use

Carfentanil is a lipophilic chemical, and can easily cross the blood-brain barrier. It has very rapid onset of effects, but is also shorter acting than fentanyl.

For pain relief, a unit of carfentanil is 100 times as potent as the same amount of fentanyl, 5,000 times as potent as a unit of heroin and 10,000 times as potent as a unit of morphine. This is despite only having 14-135 times higher affinity for the μ receptor. Despite its higher potency, carfentanil is less fatal than fentanyl in rats.
